Graduate Student Assistantship Employment Agreement  Note that interested students     must apply in order to receive a Graduate Assistance and Tuition Fellowship.  Students should     fill out this form electronically and then print it.  Please use the Student ID generated by the     University rather than your Social Security Number.

Application to Graduate This form must be turned in the semester before or near the     beginning of the semester one intends to graduate.  See the Academic Calendar for deadlines.
