• The Generic Criminal History Form will be filled out by the following Districts:

• The following Districts require specific forms to be completed:
  • Aldine ISD
  • Angleton ISD
  • Deer Park ISD
  • Fort Bend ISD
  • Harris County
  • Katy ISD
  • La Porte ISD
  • Lamar Consolidated ISD (complete the school volunteer application)
  • Pasadena ISD (also complete the Generic Criminal History Form)
  • Pearland ISD
  • Spring Branch ISD (with a copy of driver's license)
  • Texas City ISD
