To determine if you are eligible to participate in the PATH: Providing Advanced Training in Health, please complete the following checklist:

Note: The deadline for application submission is May 30, 2025 at 5:00pm CT.

Are you of Hispanic or African American descent?  Yes    No
Are you currently enrolled at UH - Main Campus, UH - Downtown, or HCC as an undergraduate student?  Yes    No
Is your current overall GPA 2.5 or greater?  Yes    No
Can you to commit to the 15-week internship program (225 hours) and all internship components?  Yes    No
 
If your answer is "Yes" to all questions, you are eligible to submit your application to PATH: Providing Advanced Training in Health internship program.
